I, the understood parent or lawful guardian of the above named person, do hereby grant my permission and consent for my child to participate in the above-mentioned Jr. Safety-at-Sea Seminar and hereby waive, release and discharge any and all claims, actions, causes of action, and suits whatsoever, in law, admiralty, or equity against the STC, STF, ChicagoYC and any affiliated companies, each of their officers, directors, employees, sponsors and agents (collectively as "Releases") resulting from or arising out of my participation in the Seminar.

I, the understood parent or lawful guardian of the above named person, grant, at no cost, the STC, STF, and ChicagoYC the absolute right and permission to use my child's, voice, image, likeness, biographical material as well as representations of the boats in any media worldwide (being television, print and internet media), including video footage, for the sole purposes of advertising, promoting, reporting and disseminating information regarding the Seminar or future seamanship related activities.
CANCELLATION POLICY: Seminar reservations may be canceled up until 1 week for a full refund. After 1 week until the event, only cancellations based on emergency needs will be considered and refunds in those cases will be 50% of the admission fee.
