I, the understood parent or lawful guardian of the above named person, do hereby grant my permission and consent for my child to participate in the above-mentioned Junior Safety at Sea Seminar on June 15, 2026 and hereby waive, release and discharge any and all claims, actions, causes of action, and suits whatsoever, in law, admiralty, or equity against the Storm Trysail Club (STC), Storm Trysail Foundation (STF), Annapolis Yacht Club (AYC) and any affiliated companies, each of their officers, directors, employees, sponsors and agents (collectively as "Releases") resulting from or arising out of my participation in the Seminar.

I, the understood parent or lawful guardian of the above named person, grant at no cost, to the STC, STF and AYC the absolute right and permission to use my child's voice, image, likeness, biographical material as well as representations of the boats in any media worldwide (being television, print and internet media), including video footage, for the sole purposes of advertising, promoting, reporting and disseminating information regarding the Seminar or future seamanship related activities.
